QIDP/Case Manager – Gillespie (Part-time) – Responsibilities included in this position: Will design, direct, and implement Individual Service Plans with functional programming and goals that are person-centered. This includes vocational goals, money management, math skills, reading and writing skills and all individualized living skills as deemed by IDT (interdisciplinary team). Maintain and record the required documentation and program reports with emphasis on the total program evaluation. Will write program plans and record necessary observations. Will help design and coordinate program activities, active treatment, staff and participant movement, and schedules. Qualifications: BA Degree in Special Education, Human Services, Psychology, Social Work, Recreation (or other degree approved by DHS); one year of experience supporting or interacting with persons with developmental disabilities. Ability to relate, communicate and work with individuals with disabilities. General working knowledge of computers, e-mail, and internet.
Registered Nurse (Back-Up) – Our agency has a need to have a nurse available on a fill-in basis to dispense meds during mid-day (or lunch hour) to our client population with developmental disabilities. Would require no more than one hour occasionally and would fill a tremendous need in assuring that meds can be dispensed on-site. Requires a valid R.N. license.
